Your joint efforts and success in helping this woman transition to independent living from an inpatient nursing facility are an example of how the California Community Transition (CCT) Project works. Our goal is to assist individuals on Medi-Cal residing in nursing facilities, and who have serious disabilities, to transition to living and receiving services in the community. Your efforts were able to bridge some of the gaps that exist in providing long-term care serivces in California, especially for younger adults. Read More »

Home & Health Care Management celebrated its 30th year in business with an official Re-Brand Launch and Ribbon Cutting Ceremony recently at their main office in Chico, California. It was well attended by the Chico Chamber of Commerce President, Board of Directors, staff and ambassadors, supportive banks and retail businesses. Also in attendance were representatives Norman Gonzales, from Congressman Tom McClintock’s office and Steve Thompson from Assemblyman Dan Logue’s office.
