Home Care Specialty Services
Medication Management: Taking medications incorrectly is one of the most common reasons people lose their independence. Our medication program utilizes computerized dispensing machines placed in your home by our licensed nurses who can also fill these machines and manage your medication regime. We have nurses available by phone 24/7 to answer your questions.
Wound Care/Management: Wounds can vary from surgical incisions, to burns, to pressure sores and abscesses. We work with your doctor to establish a wound care program to meet your individual needs. Our nurses can either provide the treatment ordered, or train a caregiver and then oversee the healing of the wound.
Injections & IVs: If your doctor has ordered injections or IV medications to be administered in your home, our nurses can help. We can administer the medication or train you or your caregiver.
Blood Draws: If mobility is an issue with getting to a lab for tests your doctor has ordered, our nurses can come to your home to draw blood and safely deliver the specimen to the appropriate lab or testing facility.
Ventilator and Respiratory Care: Managing an individual on a ventilator, or other respiratory device, at home can be intimidating and overwhelming. We can provide the support, guidance, and personalized nursing care necessary for the well-being and comfort of your loved one. Our experienced nursing team is focused on improving the quality of life for the ventilator dependent patient and their family.
In Home Fall Prevention Assessments: We want you and your loved ones to live safely at home. Our Licensed Nurses can provide an in-depth safety assessment including a home safety evaluation, an individualized patient assessment, and education on how to reduce the risk of falling. We can also recommend safety equipment/supplies to make life easier, as well as make referrals to community services.
In Home Baby Safety Assessment: A new baby in the home is a joyful occasion, but can present new challenges. Our Baby Safety Program can help alleviate your concerns. An RN will perform an evaluation to determine the safety needs for your child at home and make recommendations for the safety devices best suited to your specific home environment.
Continuous Nursing Care
Seniors, Disabled and Pediatric Patients: Our licensed and registered nurses will work with your doctor and insurance company to provide the appropriate level of nursing care up to 24 hrs a day.
Care by Home Health Aides: When physical illness and disability leads to a loss of independence we have Certified Home Health Aides to help keep your loved ones at home. We can provide bathing, personal care, and supportive activities, including meal preparation and light housekeeping for adults or children.
Case Management
Case Coordination: Our Licensed Nurses coordinate the care of individuals who, because of age, illness or disability face substantial challenges in living independently. We can develop a plan for the patient’s needs, then mobilize our resources to enhance health, safety and quality of life, while coordinating those resources through ongoing monitoring.
Referrals for community services: We work with each patient and their family to ensure that they receive the necessary community services such as (but not limited to) meals on wheels, low cost utility programs, and communication devices for the phone.
