AIDS Project Registered Nurse Case Management (RNCM)

This position requires a current and valid California RN license, two (2) years experience as an RN with at least one (1) year in a community health service and/or home health agency with supervisory experience, valid Driver’s License, and an insured automobile. Previous experience with HIV/AIDS, drug, alcohol or psych clients desirable. The AIDS Registered Nurse Case Manager provides information and education about the health and medical needs related to HIV / AIDS.

AIDS Project Social Work Case Manager

This position requires a Masters degree in social work, counseling or clinical psychology, a valid Driver’s License and an insured automobile. Previous experience with HIV/AIDS, drug, alcohol or psych clients desirable. The Social Work Case Manager provides benefits counseling and assistance with the social, emotional, and practical issues related to HIV / AIDS.

AIDS Project Case Aide

This position requires a current and valid California HHA certificate, a valid Driver’s License, current CPR certification and an insured automobile and at least two (2) years experience working as a CNA/HHA. The Case Aide functions as an integral component of the multidisciplinary case management team by assisting the case managers with practical arrangements for meeting the non-health related service needs of clients.

Certified Nurse Assistant (CNA)

This position requires a current and valid California CNA certificate (or the “Congratulations you passed the class” letter for new graduates), valid Driver’s License, and an insured automobile. We welcome experienced CNAs as well as new grads who want to work one-on-one with their clients in the client’s home. Home health allows for the CNA to really become involved in all aspects of their clients’ lives and therefore reliability and punctuality are imperative for the care of the client. The main job duties and responsibilities of a CNA are as follows:

  • Personal care (all activities of daily living)
  • Take and record vital signs (temperature, pulse, respiration, blood pressure)
  • Range of motion/therapeutic exercises
  • Report clients’ condition and any changes to their supervisor
  • Assist clients with walking, transferring and transporting the client
  • Keep client’s living area clean
  • Assist in client self-medication
  • Measure and record fluid/solid intake and output
  • Comply with all safety guidelines and regulations
  • Perform duties such as: collection of urine/stool specimens, emptying Foley catheter bags and/or other drainage reservoirs, weighing clients, testing of urine for sugar and acetone, setting up oxygen, completing unsterile dressing procedures, applying external catheters
  • Communicate effectively with his/her supervisor

Counselors (ASW, MFT, MFTI or LCSW)

For counselors with ASW, MFT, or MFTI license, this position requires a Master’s degree in psychology, social work or counseling with one year’s experience in providing direct treatment to adults, adolescents and children. Under the director of a supervising counselor, the ASW, MFT or MFTI conducts psychotherapy with clients of all ages, using various modalities including individual, family, child and group therapy to work toward improving and restoring individual and family functioning. Additional duties include completing initial assessments, diagnosis and treatment plans and completing ongoing case notes, progress and summation reports. An LCSW would require a current LCSW license and would perform the same functions without a supervising counselor.

CPR/First Aid Instructor

This position requires a current American Red Cross (ARC) CPR card and current ARC instruction card, valid Driver’s License and an insured automobile. The CPR Instructor provides both full and renewal classes to Child Care workers, healthcare workers and the general public.

Gastrostomy Trained Respite Worker (GTRW)

This position does not require any experience, however an interest in the health field is helpful. We do require the successful completion of the CPR and First Aid certification, the Gastrostomy Training Class(all provided by HHCM free of charge), valid Driver’s License and an insured automobile. Reliability and punctuality are also important because the client relies on his/her worker for many things, including but not limited to:

  • Personal Care (all activities of daily living)
  • Taking vital signs (blood pressure, temperature, respirations, pulse)
  • Measure and record fluid/solid intake and output
  • Prepare formula and give gastrostomy tube feedings
  • Keep client’s living area clean
  • Assist in transferring and transporting the client on errands
  • Assist clients with walking
  • Administer medications through the gastrostomy tube
  • Communicate effectively with his/her supervisor regarding the client.

Home Health Aide (HHA)

This position requires a current and valid California HHA certificate, valid Driver’s License and an insured automobile. We welcome experienced HHAs as well as new grads who are wanting to work in clients’ homes to provide one-on-one care. Reliability and punctuality are key aspects to success in home health because the clients rely upon their HHA with all aspects of daily living such as:

  • Personal care
  • Take and record vital signs (temperature, pulse, respiration, blood pressure)
  • Range of motion/therapeutic exercises
  • Report clients’ condition and any changes to their supervisor
  • Assist in walking, transferring and transporting the client
  • Keep client’s living area clean
  • Assist in client self-medication
  • Measure and record fluid/solid intake and output
  • Comply with all safety guidelines and regulations
  • Collection of urine/stool specimens, emptying Foley catheter bags and/or other drainage reservoirs, weighing clients, testing of urine for sugar and acetone, setting up oxygen, completing unsterile dressing procedures, applying external catheters
  • Assist clients to assume responsibilities toward self-care when possible
  • Help family members to care for the client
  • Complete monthly home study training
  • Communicate effectively with his/her supervisor

Homemaker

This position does not require any certification, but does require that you be a high school graduate, at least 18 years old, a valid Driver’s License and an insured automobile. Reliability and punctuality are also important because the client relies on his/her Homemaker for many things, including but not limited to:

  • Provides quality homemaker services in the clients homes which include but are not limited to:
    • Meal preparation
    • Shopping
    • Housecleaning (including but not limited to vacuuming, mopping, sweeping, dusting, washing dishes, cleaning showers & toilets)
    • Laundry
    • Bed making/linen changes
    • Transporting and accompanying ambulatory clients on errands and to medical appointments.
  • Reports client conditions/concerns to the RN Case Manager, Intake/Office Nurse or Scheduler.
  • Documents and maintains necessary records in an organized and neat manner.

Licensed Vocational Nurse (LVN)

This position requires a current and valid California LVN license, Driver’s License, and an insured automobile. We hire experienced LVNs as well as new graduates. (We have a special program for LVNs who have been licensed for less than one year).

Personal Care Attendant (PCA)

This position does not require any certification, but does require that you have experience in working with elderly and or disabled persons and you want to work one-on-one with a client in the client’s home. We do require the successful completion of the CPR certification, valid Driver’s License and an insured automobile. Reliability and punctuality are also important because the client relies on his/her Personal Care Attendant for many things, including but not limited to:

  • Personal Care (all activities of daily living bathing, dressing, toileting)
  • Help prepare meals, and measure and record what the person eats
  • Keep client’s living area clean by performing household cleaning services
  • Assist in transferring and transporting the client to appointments or errands
  • Assist clients with walking
  • Assist with a home exercise program
  • Report clients’ condition and any changes to their supervisor
  • Assist in client self-medication
  • Comply with all safety guidelines and regulations

Registered Nurse (RN) – Private Duty Shift Work

This position requires a current and valid California RN License, Driver’s License and an insured automobile. We hire experienced RNs as well as new graduates. We have a special program for RNs who have been licensed for less than one year.
