Our Registered Nurses and Social Workers coordinate the care of individuals who face substantial independent living challenges due to age, illness or disability. We can develop a plan for the patient’s needs, then mobilize our resources to enhance health, safety and quality of life, while coordinating those resources through ongoing monitoring.
